About Sunset Hospice
Sunset Hospice was born out of a calling to offer peace during the final stage of life. Our care team’s gentle assistance works to provide calming support to patients and families as they navigate a life-limiting illness. We support you and your family with hospice staff available 24/7 and an integrated care plan that includes emotional, physical, social, financial, and bereavement support.

Hospice nurses available 24/7
Each hospice care plan includes regular registered nurse (RN) visits to manage symptoms, review medications, and ensure ongoing comfort with 24/7 on-call nurse availability for urgent needs. Whether care is provided at home or in a residential community, our team offers immediate support, reassurance and attentive guidance whenever it’s needed most.
Short-term inpatient and respite care
Short-term inpatient and respite care offer families additiona support when symptoms become difficult to manage at home or when caregivers need time to rest. When appropriate, we help coordinate care in a qualified facility to ensure comfort, safety, and continuous monitoring. Our team stays closely involved to support both the patient and their loved ones throughout the stay.
Concierge pharmacy services
Our concierge pharmacy services make medication management simple and stress-free for families. We coordinate presriptions, handle refills, and ensure timely delivery of hospice-approved medications for pain and symptom relief. Under close oversight from our hospice physician, each patient receives the right medication at the right time – allowing families to focus on being together instead of managing logistics.
Family counseling
Our family counseling services provide emotional support, anticipatory grief guidance, and practical coping tools for loved ones. Through our licensed social workers and hospice counselors, families receive compassionate assistance in understanding changes, navigating difficult decisions, and finding strength throughout each stage of the journey.
Social & spiritual services
We honor every family’s unique beliefs, values, and traditions. Our social workers and spiritual conselors offer emotional guidance, support specific cultural or religious practices, and provide comfort through prayer, reflection, or life-review conversations. We’re here to help patients and families find strength, meaning, and peace during this important time.
Regular health assessments
We provide regular health assessments to ensure each patient’s comfort, symptoms, and needs are closely monitored. Our hospice team works together to adjust care plans, manage medications, and respond quickly to any changes – always prioritizing comfort, dignity and quality of life.

Volunteers at Sunset Hospice Inc. provide companionship, emotional support, and meaningful activities that brighten each patient’s day. Bereavement support
We offer bereavement support for upt to 13 months to help families navigate grief with understanding and compassion. Through regular check-ins, conseling resources, and emotional and spiritual guidance, we stand beside famiilies as they reflect, adjust, and find renewed strength in the months ahead.
