We offer four levels of hospice care:

at- home care

Routine Home Care

We offer routine home care wherever the individual calls home, including both nursing homes and assisted living facilities.

continuous care

Continuous Home Care

During times when the pain or symptoms become too great, the Sunset team of medical providers and staff will expertly manage the patient’s symptoms in an effort to keep individuals at home.

respite care

Inpatient Respite Care

When primary caregivers need to take a break, we provide inpatient respite care in an approved Medicare facility for up to five nights at a time.

General Inpatient Care

This type of care takes place in an approved Medicare facility, including hospitals and skilled nursing centers. It is an option when the patient needs specialized care to relieve symptoms.

Oversees medical care and prescribes non-curative treatment to manage any pain or symptoms.

Liaison between the attending physician, patient, and family, and provides information and insight into the patient’s unique and changing needs.

RN Case Manager (RN)

Responsible for coordinating all aspects of the integrated plan for patient care while overseeing symptom control and pain management and administering medications as needed.

Social Worker

Advises patients and families when needed, develops an integrated care plan based on emotional, physical, social, and financial needs, and assists with bereavement support after a loved one has passed.

Spiritual Counselor

Draws on the beliefs and values of patients and families to cope with the spiritual aspects of illness and loss. Can assist with specific religious needs and lead memorial services.
