Full Job Description

Sunset Hospice is currently seeking tow RN Case Managers to join our West Houston Team with service areas: Katy, Fulshear, Richmond, Sugarland, Brookshire, Sealy.

Responsibilities:

The RN Case Manager, in collaboration with the entire Interdisciplinary Team, is responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating the strategies and services required to meet a patient/family’s end-of-life care needs. An outcome-based plan of care is created and continually adjusted through utilization of the nursing process, relationship centered care principles, and holistic philosophy, assuring that the changing needs of the patient/family are met while facilitating a pain-free, peaceful death. Demonstrates personal accountability for own continuous growth and development. Seeks out appropriate opportunities to enhance competency and improve performance.

Requirements:

Current license to practice as Registered Nurse in Texas required. Minimum one year experience in direct patient care; hospice experience or long term care experience is preferred. Visits in private homes and/or facilities. Work schedule is variable according to patient and family needs. Works on an on-call basis through a set rotation. Strongly encourage CPR certification; valid driver’s license and proof of insurance required.
